Driving Safety Class Liability Waiver and Release Form
Program Description:
The Proactive Safety and Security Driving Safety Class includes interactive activities, including the use of impairment simulation goggles to simulate the effects of driving under the influence. These activities may involve mobility exercises that can cause disorientation, balance issues, and an increased risk of falling or injury.
Acknowledgement of Risk:
Impairment goggles will be used during the class, which may cause loss of balance, dizziness, and disorientation.
Participation in the practical portion of this class involves inherent risks, including but not limited to slips, falls, collisions, or other injuries.
It is my responsibility (or the responsibility of my parent/guardian if under 18) to inform the instructor prior to participation if I do not wish to engage in the practical activity or if I have any medical conditions, physical limitations, or concerns that may increase the risk of injury.
Participation in the practical portion is voluntary.
Release of Liability:
By signing below, I hereby release, waive, and discharge Proactive Safety and Security LLC, its owner, employees, instructors, affiliates, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur as a result of my participation in this class.
I certify that I am in good physical condition and have no known conditions that would prevent safe participation, or I have disclosed such conditions to the instructor.*
